Consent for Use of Photographs/Videos

I give my permission/consent for the CFDN charity to use my photograph(s) and/or video recordings for the purposes of advertising, fundraising, marketing, social media, publications, updating funders and promotional materials in furthering the charity and its activities.

I understand that I can withdraw my consent at any time by contacting the CFDN in writing via the above details.

For Members OVER 18 Years:

I confirm that I am over 18 years old or have obtained parental/guardian consent (if under

18).

For members UNDER 18:

As the parent/guardian, I give permission for the CFDN to use images of my child for the above stated reasons in helping to further the charity.

This form ensures compliance with privacy laws and gives members control over their image use. A form must be completed for each member.
